Milestones As Of August, 2010
Community Outreach With The Mobile Multi-Service Center (MMSC)
- Last year, Illumination Foundation, in partnership with OC Health Care Agency, organized 45 MMSC clinics at 19 sites and served 970 clients
- In 2010, 11 MMSC clinics provided 1388 patient services to an estimated 1083 clients
Interim Housing And Wrap-Around Program For Homeless Families With Children
- Since July 2008, the Interim Housing Program served 165 families, consisting of 348 children and 244 adults
- 74 families graduated from the program into stable housing
- 53% of the adults currently in the program secured employment
- 22% of the adults currently in the program are continuing their education
- 67% of children currently in the program received accolades at school
Permanent Housing Placement For Homless Or At-Risk Families And Individuals
- Since October 2009, Illumination Foundation transitioned 58 households into permanent housing
Recuperative Care For Homeless Discharged Patients
- IF's Recuperative Care Program has saved OC hospitals $1.6 million in 6 months
- Since January 2010, the Recuperative Care Program served 101 clients for a total of 962 nights
- Illumination Foundation connected 74% of these homeless recuperative clients with housing
