  • Community Outreach Programs

    The Mobile Multi-Service Center (MMSC)

    Travels throughout Orange County providing free medical care and wraparound services to the homeless to:

    1. Help those at risk of homelessness before they reach a crisis situation
    2. Connect the homeless with services to help them reach self-sufficiency

    Family is Home Partnership Program

    This program provides dedicated outreach efforts and case management to families living in motels throughout Orange County.

  • Interim Housing Program

    Bridge the gap between emergency shelters and permanent housing to provide Interim Housing and comprehensive services to underserved homeless families with children.

  • Permanent Housing Program (HPRP)

    Address the economic root cause of homelessness in Orange County by transitioning families and individuals into affordable permanent housing to facilitate long-term stability.

    A Partnership With National Health Foundation

    Manage the discharge of homeless patients from hospitals and help them successfully transition back into the community.
