Recuperative Care Program
Benefit to the Community
Manage the discharge of homeless patients from hospitals to help them successfully transition back into the community
2011 Goals
- Provide at least 7,200 Recuperative Care bed nights (2,400 in Orange County and 4,800 in Los Angeles County) and connect at least 50% of clients to housing in Orange County and 25% in LA County
Program Activities
- Ensure all clients receive ongoing RN oversight, case management, medical care and behavioral health counseling
- Connect clients to resources designed for their specific needs
- Provide basic needs including shelter, food, hygiene supplies and transportation
- Connect clients to a medical home, introduce them to appropriate community services and connect them to housing if possible
