Permanent Supportive Housing

Benefit to the Community

Transition families and individuals into affordable permanent housing to facilitate long-term stability

2011 Goals

  • Place 100 households in permanent supportive housing and ensure 80 families maintain housing for at least 6 months

Program Strategies

  • Create a safety net for newly homeless families at risk of spiraling downward into chronic homelessness
  • Prevent families at risk of homelessness from becoming homeless
  • Provide case management and wraparound services to clients to support their transition into permanent affordable housing
  • Screening, Intake and Needs Assessment: Develop an action plan including short- and long-term goals and objectives for each client
  • Permanent Housing: Assist families in moving into permanent, affordable housing by helping them overcome barriers to obtaining permanent housing
