Community Outreach Programs

The Mobile Multi-Service Center (MMSC)

Travels throughout Orange County providing free medical care and wraparound services to the homeless to:

  1. Help those at risk of homelessness before they reach a crisis situation
  2. Connect the homeless with services to help them reach self-sufficiency

Family is Home Partnership Program

This program provides dedicated outreach efforts and case management to families living in motels throughout Orange County.

2010 Goals

  • Coordinate weekly Mobile Multi-Service Center clinics in different locations throughout Orange County
  • Assist 20 families who are at-risk of becoming homeless to secure permanent housing
  • Connect 40 homeless or at-risk individuals with job-training services
  • Connect 800 homeless individuals with medical care through mobile medical vans

Broad Plan of Action

  • Determine at-risk status of program beneficiaries using factors recommended by the National Alliance to End Homelessness
  • Outreach efforts will identify those who are at-risk but are reluctant to seek assistance: Collaborate with Orange County Health Care Agency, police forces, school districts, and faith-based organizations to create a broad safety net.
  • Housing Specialist will either assist the families in staying in their current home or relocating to alternative affordable-housing,
  • Workforce Specialist will provide job training, employment opportunities and financial literacy
  • Medical Vans will provide medical services on-site
    • Medical, dental, vision, mental health services
    • Connect clients to medical and socio-economic benefits
    • Referrals to community clinics, physicians, CalOPTIMA
    • Outcome data collection
  • Public Law Center to alleviate legal issues that prevent homeless individuals from securing employment and reaching self-sufficiency
